📖 SYNOPSIS
Two strangers on their first date, Aki and Kazuo, are hitting it off just fine. Things are going well — nervous energy, awkward small talk, the whole deal. Then they wake up chained in a basement with a man who calls himself a doctor standing over them. That's the setup, and it doesn't get lighter from there.
Grotesque is a CloverWorks original movie arriving in 2026, and it's drawing from the extreme end of horror. Think less supernatural dread, more raw human cruelty in a confined space. The entire thing takes place in what amounts to a single room, and that claustrophobia does a lot of heavy lifting. There's nowhere to look away, for the characters or for you.
The core tension isn't really about whether they survive — it's about what this sadistic captor wants from them and whether their connection to each other means anything when everything else is stripped away. It's psychological horror wearing a torture horror skin.
If you've seen films like Saw or Hostel and thought anime could push into that territory with its own visual language, this is exactly that. Fans of the Guinea Pig series will recognize the DNA here too. CloverWorks handling this kind of material is an interesting choice — they're known for polished, emotional work, which could make the darker moments hit even harder.
Fair warning: this one is not for the squeamish. At all. But if you want horror anime that commits fully to being uncomfortable, keep an eye on it.
